    Towards a Reference Architecture for Context-Aware Services

    This Chapter describes an infrastructure for multi-modal perceptual systems which aims at developing and realizing computer services that are delivered to humans in an implicit and unobtrusive way. The framework presented here supports the implementation of humancentric context-aware applications providing non-obtrusive assistance to participants in events such as meetings, lectures, conferences and presentations taking place in indoor "smart spaces". We emphasize on the design and implementation of an agent-based framework that supports "pluggable" service logic in the sense that the service developer can concentrate on the service logic independently of the underlying middleware. Furthermore, we give an example of the architecture’s ability to support the cooperation of multiple services in a meeting scenario using an intelligent connector service and a semantic web oriented travel service. The framework was developed as part of the project CHIL (Computers in the Human Interaction Loop). The vision of CHIL was to be able to provide context-aware human centric services which will operate in the background, provide assistance to the participants in the CHIL spaces and undertake tedious tasks in an unobtrusive way. To achieve this, significant effort had to be put in designing efficient context extraction components so that the CHIL system can acquire an accurate perspective of the current state of the CHIL space. However, the CHIL services required a much more sophisticated modelling of the actual event, rather than simple and fluctuating impressions of it. Furthermore, by nature the CHIL spaces are highly dynamic and heterogeneous; people join or leave, sensors fail or are restarted, user devices connect to the network, etc. To manage this diverse infrastructure, sophisticated techniques were necessary that can map all entities present in the CHIL system and provide information to all components which may require it. From these facts, one can easily understand that in addition to highly sophisticated components at an individual level, another mechanism (or a combination of mechanisms) should be present which can handle this infrastructure. The CHIL Reference Architecture for Multi Modal Systems lies in the background, and provides the solid, high performance and robust backbone for the CHIL services. Each individual need is assigned to a specially designed and integrated layer which is docked to the individual component, and provides all the necessary actions to enable the component to be plugged in the CHIL framework

    CXCL13 and CXCL9 CSF Levels in Central Nervous System Lymphoma-Diagnostic, Therapeutic, and Prognostic Relevance

    Background: Diagnostic delay and neurologic deterioration are still a problem for the treatment of rapidly progressing CNS lymphoma (CNSL); there is an unmet need for a diagnostic test with a high diagnostic yield and limited risk, minimizing the time to the initiation of effective treatment. Methods: In this prospective monocentric study, we analyzed the utility of CXCL13 and CXCL9 as diagnostic, therapeutic and prognostic biomarkers for CNSL. Cerebrospinal fluid (CSF) from 155 consecutive patients admitted with brain lesions of various origins was collected. Levels of CXCL13 and CXCL9 were analyzed by ELISA. Additionally, CSF was analyzed during CNSL disease course (relapse, remission, progress) in 17 patients. Results: CXCL13 and CXCL9 CSF levels were significantly increased in patients with CNSL compared to control patients with lesions of other origin. Using logistic regression and a minimal-p-value approach, a cut-off value of 80 pg/ml for CXCL13 shows high sensitivity (90.7%) and specificity (90.1%) for the diagnosis of active CNSL. CXCL9 at a cut-off value of 84 pg/ml is less sensitive (61.5%) and specific (87.1%). Both cytokines correlate with the clinical course and response to therapy. Conclusions: Our results confirm the excellent diagnostic potential of CXCL13 and introduce CXCL9 as a novel albeit less powerful marker for PCNSL

    Decline in the number of patients with meningitis in German hospitals during the COVID-19 pandemic

    BACKGROUND AND OBJECTIVES: In 2020, a wide range of hygiene measures was implemented to mitigate infections caused by the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). In consequence, pulmonary infections due to other respiratory pathogens also decreased. Here, we evaluated the number of bacterial and viral meningitis and encephalitis cases during the coronavirus disease 2019 (COVID-19) pandemic. METHODS: In a multicentre retrospective analysis of data from January 2016 until December 2020, numbers of patients diagnosed with bacterial meningitis and other types of CNS infections (such as viral meningitis and encephalitis) at 26 German hospitals were studied. Furthermore, the number of common meningitis-preceding ear-nose-throat infections (sinusitis, mastoiditis and otitis media) was evaluated. RESULTS: Compared to the previous years, the total number of patients diagnosed with pneumococcal meningitis was reduced (n = 64 patients/year in 2020 vs. n = 87 to 120 patients/year between 2016 and 2019, all p < 0.05). Additionally, the total number of patients diagnosed with otolaryngological infections was significantly lower (n = 1181 patients/year in 2020 vs. n = 1525 to 1754 patients/year between 2016 and 2019, all p < 0.001). We also observed a decline in viral meningitis and especially enterovirus meningitis (n = 25 patients/year in 2020 vs. n = 97 to 181 patients/year between 2016 and 2019, all p < 0.001). DISCUSSION: This multicentre retrospective analysis demonstrates a decline in the number of patients treated for viral and pneumococcal meningitis as well as otolaryngological infections in 2020 compared to previous years. Since the latter often precedes pneumococcal meningitis, this may point to the significance of the direct spread of pneumococci from an otolaryngological focus such as mastoiditis to the brain as one important pathophysiological route in the development of pneumococcal meningitis.     Diagnostic Value of Lumbar Facet Joint Injection: A Prospective Triple Cross-Over Study

    The diagnosis “lumbar facet syndrome” is common and often indicates severe lumbar spine surgery procedures. It is doubtful whether a painful facet joint (FJ) can be identified by a single FJ block. The aim of this study was to clarify the validity of a single and placebo controlled bilateral FJ blocks using local anesthetics. A prospective single blinded triple cross-over study was performed. 60 patients (31 f, 29 m, mean age 53.2 yrs (22–73)) with chronic low back pain (mean pain persistance 31 months, 6 months of conservative treatment without success) admitted to a local orthopaedic department for surgical or conservative therapy of chronic LBP, were included in the study. Effect on pain reduction (10 point rating scale) was measured. The 60 subjects were divided into six groups with three defined sequences of fluoroscopically guided bilateral monosegmental lumbar FJ test injections in “oblique needle” technique: verum-(local anaesthetic-), placebo-(sodium chloride-) and sham-injection. Carry-over and periodic effects were evaluated and a descriptive and statistical analysis regarding the effectiveness, difference and equality of the FJ injections and the different responses was performed. The results show a high rate of non-response, which documents the lack of reliable and valid predictors for a positive response towards FJ blocks. There was a high rate of placebo reactions noted, including subjects who previously or later reacted positively to verum injections. Equivalence was shown among verum vs. placebo and partly vs. sham also. With regard to test validity criteria, a single intraarticular FJ block with local anesthetics is not useful to detect the pain-responsible FJ and therefore is no valid and reliable diagostic tool to specify indication of lumbar spine surgery. Comparative FJ blocks with local anesthetics and placebo-controls have to be interpretated carefully also, because they solely give no proper diagnosis on FJ being main pain generator

    High-level information management in joint ISR based on an object-oriented approach

    We present research with regard to a world modeling component called Object-Oriented World Model (OOWM) in order to perform High-Level Information Management (HLIM) in Joint ISR (Intelligence, Surveillance, and Reconnaissance). The OOWM allows to structure a priori available domain knowledge as well as current information gathered, e.g., according to specific collection and exploitation tasks, in an object-oriented way, i.e., by storing all available information in a structure which explicitly represents state, properties and relations of the relevant objects in the environment. It serves as a central information hub for HLIM components like High-Level Data Fusion (HLDF) modules and, by the employment of a domain model, it allows establishing a common understanding of terms. The OOWM concept supplements the CSD (Coalition Shared Data) concept which is the agreed state-of-the-art approach for interoperable information sharing in Joint ISR. In this publication, a special focus is placed on using current information given in form of formal exploitation reports. Based on this, we also present a new concept called Object-Oriented Reporting (OOR) which corresponds to a version of formal reporting being even more suitable for and closely related to the OOWM approach than current formal exploitation reports already are

    Six-layer lamination of a new dry film negative-tone photoresist for fabricating complex 3D microfluidic devices

    We present a new epoxy-based negative-tone dry film photoresist (DFR) for fabricating multilayer microfluidic devices using a lamination process combined with a standard photolithography technology. As proof-of-concept, a complex 3D-hydrodynamic focusing device was produced via a six-layer lamination process of 33 µm-thick DFR layers. The bonding strength of the new DFR was tested on silicon, glass, and titanium substrates, respectively. A maximum bonding strength of 37 MPa was obtained for the dry film photoresist laminated on glass. No leakage was found, and burst tests proved excellent robustness and sealing reliability of the microchannels
